Governance is the product now
Agentic AI is moving from answers to actions. The control layer is the hard part.
Over the last 18 months, most leadership conversations about AI have focused on model choice and estimated productivity gains. With the advent of Opus 4.5 and GPT-5.2, those boundaries continue to shift.
But the next stage is bigger, and riskier: AI systems that do not merely generate content, but plan, take actions and execute across tools, teams and business functions.
The conversation changes from “What does the model say?” to “What can the system do?”
The UK Information Commissioner’s Office published a Tech Futures report on agentic AI on 8 January 2026. It is foresight rather than formal guidance, but it signals the direction of travel for organisations building or adopting agentic workflows. One observation in the report matters particularly for leaders:
Design and architectural choices determine whether agentic AI becomes a trust accelerator—or a risk amplifier.
Once an agent can access systems, data and tools, the blast radius is no longer confined to one bad answer. It can mean:
- A wrong record being updated.
- A transaction being triggered.
- Sensitive information being pulled into the wrong context.
- A chain of automated decisions becoming difficult to unwind.
So what changes for CEOs, CIOs, COOs and CROs?
Three realities to plan for
1. Agents turn digital and data risk into operational and financial risk
When AI can act, the failure mode is no longer simply misinformation. It is execution: changes made, workflows triggered and permissions used.
2. Accountability gets harder as the value chain gets longer
Agentic systems often span multiple tools, vendors and components. If something goes wrong, leaders still need to answer two questions: who is responsible, and what evidence proves it?
3. Trust becomes an engineering discipline, not a communications strategy
Customers, employees and regulators will demand:
- Visibility into what happened.
- The ability to contest or correct it.
- Proof that controls were in place, not merely promises.
What “good” looks like: agentic controls
The ICO report highlights the need for practical governance mechanisms: monitoring, auditing, permission structures, authentication, data-access protocols, and routes for redress and restitution.
In plain terms, agentic AI needs a control plane.
It is not enough to have an agent that can do things. The operating layer must answer:
- What may the agent access?
- What may it change?
- Where must a human approve?
- What is logged, and can the full chain of actions be reconstructed?
- How do we stop, roll back and remediate?

If you are exploring agentic AI in 2026, this pattern is a useful place to begin:
- Pick one important workflow—high value or high volume—with manageable downside.
- Define its boundaries: objectives, permissions, data and tools.
- Insert human checkpoints at irreversible decisions.
- Log the process end to end, from inputs through actions to outputs.
- Run in shadow mode before allowing the agent to write to production.
Then expand on evidence, not enthusiasm.
